Ganglion Cyst

WHAT IS A GANGLION CYST?

Ganglion Cyst (Synovial Cyst)

A Ganglion Cyst is a benign, mucin-filled fluid sac that arises from adjacent joint capsules or tendon sheaths. Most commonly occurring around the wrist and hand, these smooth subcutaneous nodules are non-cancerous and cannot spread. Cyst dimensions often fluctuate, expanding with joint exertion or spontaneously remitting.

Ganglion Kisti Belirtileri Nelerdir?

Ganglion cysts most commonly develop along the dorsal or volar aspect of the wrist and flexor tendon sheaths. They range from pea-sized swellings to masses exceeding 2.5 cm. Repetitive joint mobilization typically increases cyst fluid accumulation. While generally painless, compression of adjacent sensory nerves may cause localized aching, tingling, or diminished grip strength.

What Causes a Ganglion Cyst?

Ganglion cysts typically develop from micro-trauma or focal capsular degeneration. Connective tissue breakdown stimulates production of thick hyaluronic fluid that herniates through a capsular stalk to form an expanding subcutaneous cyst.

Risk Factors & Demographics

Ganglion cysts occur predominantly between 20 and 40 years of age and are three times more prevalent in females. Individuals whose occupations or sports impose repetitive wrist loading (such as gymnasts) and those with localized joint sprains or osteoarthritis have a higher predisposition.

Diagnostic Evaluation

Diagnosis is established through clinical examination, palpation, and transillumination. High-resolution ultrasound or MRI confirms the internal cystic nature, delineates vascular anatomy, and rules out occult masses. In-office aspiration provides immediate diagnostic and therapeutic relief.

Treatment Modalities

Asymptomatic ganglion cysts typically warrant watchful observation. When painful or functionally restrictive, treatment options include immobilization, needle aspiration, or surgical excision.

A prefabricated wrist brace can alleviate mechanical stress and assist fluid reabsorption. However, prolonged immobilization is avoided to prevent muscular disuse and joint stiffness. For persistent cysts, outpatient fine-needle aspiration of the viscous mucinous fluid decompresses the cyst. A localized corticosteroid dose may be instilled to reduce synovial inflammation. Because aspiration leaves the underlying capsular stalk intact, recurrence is common, in which case definitive surgical excision is recommended.

Surgical excision (open or arthroscopic ganglionectomy) removes the cyst together with its capsular pedicle and underlying synovial base, significantly minimizing recurrence while preserving joint function.
