Tailbone (Coccyx) Pain

PROF. DR. YAVUZ KOCABEY

WHAT IS TAILBONE (COCCYX) PAIN (COCCYDYNIA)?

Tailbone (Coccyx) Pain (Coccydynia)

Coccydynia refers to localized, persistent pain and tenderness around the coccyx (tailbone) at the base of the spine, exacerbated particularly while sitting or rising from a seated position. The coccyx is a terminal triangular bone forming the base of the spinal column and serving as an insertion site for pelvic floor muscles and ligaments. Irritation, subluxation, hypermobility, or chronic sprain in this region leads to debilitating discomfort that requires accurate anatomical evaluation.

Tailbone (Coccyx) Pain (Coccydynia) Nedenleri

Childbirth Strain: During vaginal delivery, the passage of the fetal head—particularly with higher birth weights—can hyper-extend or sprain the sacrococcygeal joint ligaments, leading to persistent postpartum coccyx pain.

Direct Trauma: Direct impact from a backward fall onto a hard surface or stairs can cause coccygeal contusion, subluxation, or fracture.

Repetitive Micro-Stressors: Prolonged sitting on rigid surfaces, repetitive vibration, or sporting activities such as bicycling, horse riding, or rowing that cause continuous mechanical pressure on the coccyx.

Diagnostic Methods

Clinical Physical Examination: Point tenderness directly upon digital palpation and gentle mobilization of the coccyx confirms localized inflammation.

Diagnostic Imaging: Dynamic dynamic standing/seated X-rays and high-resolution pelvic/coccygeal MRI evaluate hypermobility, subluxation, bone marrow edema, or occult fracture.

Treatment Modalities for Tailbone Pain (Coccydynia)

  1. Conservative Management
  2. Coccygeal Cushion & Pharmacotherapy
  3. Fluoroscopy-Guided Corticosteroid Injections: Accurate needle placement is vital. Infiltration is performed under real-time fluoroscopic (C-arm) guidance directly at the inflamed sacrococcygeal joint or ganglion impar.
  4. Surgical Coccygectomy

Tailbone pain stems from distinct mechanical and structural causes. Accordingly, our therapeutic pathway is tailored to the specific origin and chronicity of the pain.

Diagnostic clarity is achieved through focused digital palpation and targeted dynamic imaging (dynamic X-rays and MRI) to verify coccygeal mobility and exclude occult fractures or bone edema.

Short-term targeted anti-inflammatory pharmacotherapy and non-steroidal analgesics help reduce acute pericoccygeal inflammation and relieve severe sitting discomfort.

Specialized physical therapy focused on pelvic floor relaxation, manual sacrococcygeal mobilization, and postural education effectively relieves ligamentous tension.
